Short-Term Rehabilitation Care
As a leader in rehabilitation services, The Ambassador Lincoln is ever mindful that our patients are people first - and our approach to care reflects this core belief. It begins with a careful evaluation of the patient and medical history, along with discussions with the patient and family, physicians, and other medical providers. From there, we develop an individualized, comprehensive care plan that is focused on making the transition from hospital to rehab facility to home. Built on a wellness concept designed to further enhance activities of daily living, our multi-disciplinary approach brings together a combination of core team members. Your individualized, multi-disciplinary team may include: physical, occupational, speech, and respiratory therapists, skilled nursing services, social services, discharge planning, a registered dietician, and an individualized activity program.
Making the decision to move to a long-term care facility can be difficult and overwhelming. The Ambassador Lincoln makes the transition from home comfortable and seamless. Our team of dedicated staff work together to ensure our residents feel safe, secure, and welcome in their new home. For more than 25 years, The Ambassador Lincoln Pulmonary Unit has provided the region’s premier Pulmonary Special Care Program. Staffed 24 hours a day by registered nurses and licensed respiratory therapists, the unit operates under the leadership of a Board-Certified Pulmonary Physician.
Following a carefully developed care plan, our pulmonary staff helps to ensure our pulmonary residents reach their highest individual potential. Short-term weaning candidates are treated aggressively, while long-term patients are given a slower approach. Yet, in every case, our ultimate goal is to enable the resident to gain freedom from ventilator support and preserve the dignity and quality of life while under our care.

Physical Therapy
At the center of our physical therapists' care is the restoration of function and the prevention of disability resulting from disease, injury, or illness. They enable patients to strengthen muscles and encourage the return of motion through a variety of therapies and exercises.
Occupational Therapy
Occupational Therapists provide intervention to minimize physical deficits that interfere with the performance of activities and tasks of daily living. Emphasizing self-care and community activities, they help to achieve maximum independence and enhance the quality of life for individuals in our care.
Speech Therapy
Speech Therapists help individuals to regain and improve critical communication, cognition, and swallowing skills to the patient's highest possible functional level.
Repiratory Therapy
The primary role of the respiratory therapist is to provide treatments and interventions to allow optimal pulmonary function. They focus on helping individuals to recover from respiratory illnesses.
Dietary Servcies
Healthy diets and eating patterns can significantly impact an individual's ability to enjoy a better quality o flife. Through The Ambassador Lincoln's dietary services, a registered dietician provides therapeutic diets designed to reflect the varying ages and dietary needs of the residents we serve.
Skilled Nursing Servcies
Skilled Nursing Care is provided by Registered and Licensed Practical Nurses. Each of The Ambassador Lincoln's specialized units is under the direction of a Registered Nurse whom oversees the development and implementation of each resident's personalized plan of care.
Social Servcies/Discharge Planning
The expertise of the social services staff allows them to assist patients, residents, and families with the transition from the acute care setting. They act as an advocate for patients, residents, and families in the coordination of all aspects of their care and arrange the appropriate services when transitioning to their next level of care.
Activity Program
Meeting the social needs of each patient and resident is a primary goal of the activities program. A wide variety of both individual and group activities are offered to enhance the patients’ and residents’ overall emotional well-being.
