Resident Council

One way to exercise your rights as a resident is through the Resident Council. We hope you will take an active part in our Resident Council and attend the monthly meetings which are held the third Monday of each month.
 
The Resident Council is an organization of The Ambassador residents whose purpose is to provide a means to share concerns, problems and ideas with other residents and staff members. It also allows you and other residents the opportunity to participate in the affairs and decisions that influence your life each day while you are a resident here.  Resident Council is led by elected residents of the facilty and is supervised by the social work department. Resident Council also appoints special committees as necessary to carry out special purposes and programs of the council. The Council is YOUR voice in this facility and aims at responding to YOUR needs and interests.

 



Family Forum

Family Forum is held on a quarterly basis.  At this time, The Ambassador arranges speakers to present topics of concern, such as issues on Advance Directives, Alzheimer's Disease, etc. This is the time where family members can support one another and voice any grievances/concerns to staff.  If your family has ideas or topics for discussion in which you would be of interest, please contact the Social Work department so arrangements can be made. 

  



Resident Grievance Procedure

Any resident or resident's representative may, at anytime, register a complaint or grievance with the Facility Administrator or his designee regarding any aspect of the operation of the Facility or the care of the resident. The complaint may be oral or in writing and need not be signed by a resident. All complaints shall be acted upon within four working days of receipt. A written description of the action shall be retained in the Facility files. The Facility maintains a record of all complaints. Grievance forms are available at the nurses' station, business office, social work office, and in the Administrator's office. The Ambassador Group Inc. is responsible for the management and quality assurance of this facility. 

If for any reason your concerns or problems are not being resolved at the facility by the Administrator or his/her staff, we would like the opportunity to resolve differences or concerns. Please call collect or write to us.

Transfer Policy

Transfer is the movement of the Resident from one location to another within the Facility. The Resident may be asked to transfer between Facility sections due to a change in care needs, medical reasons or financial reasons. Transfers will be conducted in accordance with the procedures outlined in the most recent NHCA Resident Rights Handbook.

 



Involuntary Discharge
Notice may be given as soon as practical prior to discharge when:

 

  • An immediate discharge is required by the Resident's urgent medical needs as documented by a physician;
  • The health or safety of the Resident, other Resident's and/or employees in the Facility is endangered; and
  • The Resident's health improves sufficiently to allow a more immediate discharge.  

A 10-day written notice will be given to the Resident's responsible party prior to involuntary discharge of the resident for five or more days in arrears of payment.  Involuntary transfers and discharges will be conducted in accordance with the procedures outlined in the most recent NHCA Resident Rights handbook.      
 



Resident Responsibilities

Resident's are responsible for the following:

 

  1. Providing, to the best of his/her knowledge, accurate and complete information about present complaints, past illnesses and hospitalizations, medications, and other matters relating to his/her health.
  2. Reporting unexpected changes in his/her condition to the responsible practitioner.
  3. Making it known whether he/she clearly comprehends a contemplated course of action and what is expected of him/her.
  4. Following both the treatment plan recommended by the practitioner primarily responsible for his/her care and the organization's rules and regulations affecting resident care and conduct. This includes following the instructions of nurses and other health care professionals as they carry out the plan of care and implement the responsible practitioner's orders and as they enforce the applicable organization rules and regulations.
  5. Understanding the consequences if treatment is refused or if the practitioner's instructions are not followed.
  6. Being considerate of the rights of other residents and organization personnel and responsible for his/her behavior in the control of noise, smoking and number of visitors.
  7. Being respectful of the property of others and of the organization.
  8. Assuring that the financial obligations for his/her health care are fulfilled as promptly as possible. 

 
Overall Plan of Care

The resident has an individualized, written plan of care based on his/her individual needs, problems and strengths.  The resident and his/her family will be informed of the date and time of the evaluation and will be encouraged to attend. These meetings will be held at least quarterly.
